IDENTIFICATION NUMBERS
NOW REQUIRED ON VOTER REGISTRATION APPLICATIONS

City Commissioner’s Chairwoman Margaret M. Tartaglione, Commissioner Anthony Clark, and Commissioner Joseph J. Duda urgently request that all Philadelphians take note of changes to Federal and State laws regarding applications to register to vote.

Effective January 1, 2006, all applications to register to vote MUST include the following:

• You MUST supply a Driver's License Number or Pennsylvania Non Driver's Photo ID Number, if you have one.

• If you do not have a Driver's License Number or Pennsylvania Non Driver's Photo ID Number, you MUST supply the last four digits of your Social Security Number.

• If you do not have a Social Security Number, please write "None" in the box.

The Pennsylvania Voter Registration Mail Application form, available at Post Offices, State Liquor Stores, Free Library Branches, and our office locations, includes boxes on the form for an applicant to provide the above required numbers.
